CountyCare covers medically necessary healthcare services, including doctor visits, hospital stays, labs, X-rays, vision and dental coverage, online services for expecting parents and personalized healthcare with no copays.

As a CountyCare member, your covered prescription drugs are provided at no cost to you when you get your prescriptions filled at an in-network pharmacy. Our pharmacy network includes several national retail chains such as CVS, Kmart, Walgreens, Meijer, Osco, Target, Walmart, and independent pharmacies. Make sure you have your CountyCare member ID card to show at the pharmacy.
If the primary care provider (PCP) listed on your card is not correct, or you want to switch doctors, you can do so at any time by contacting Member Services at
312-864-8200/ 855-444-1661 (toll-free) / 711 (TDD/TTY). You can also change your PCP in the member portal or submit a PCP Change Request Form online.
Open enrollment happens once a year on your anniversary date and is when you choose which Medicaid health care plan you want. Open enrollment is for people who have already been approved or are already enrolled in Medicaid. Good news! If you want to stay with CountyCare, you don’t need to do anything.
Redetermination, or renewal, is the annual process when the State of Illinois confirms if you are eligible to receive Medicaid services. If you also receive SNAP benefits, you may have to renew every six months.
