About the Survey
The Consumer Assessment of Healthcare Providers and Systems (CAHPS) survey is your opportunity to tell us what you think about the services we provide at CountyCare. The survey is part of a national project by the National Committee for Quality Assurance (NCQA), a non-profit group that helps people learn more about health plans.

How to Take the Survey
If you are selected to take the survey, you will receive a letter in the mail that looks like this along with a copy of the survey. Collection of survey responses will only occur during Spring 2026.
- Complete the paper survey you receive in the mail.
- Return it using the postage-paid envelope provided.

- Go to the website provided or scan the QR code to complete the survey online.
- You must use the username and password provided in the letter.
- It’s quick, convenient and secure.
