Important Reminders & Key Updates

An update regarding CountyCare’s updated NCQA accreditation:

Roster Reminder

Keep Your Roster Information Updated

The Illinois Association of Medicaid Health Plans (IAMHP) updated the Universal Provider Roster template in May 2026. As a reminder, all managed care organizations (MCOs), including CountyCare, accept this template.

CountyCare requires providers to submit a comprehensive roster quarterly and report any changes or updates monthly using the most current version of the Universal Provider Roster template.

Completed rosters should be emailed to [email protected].

For questions or assistance, contact your assigned Provider Relations representative, call Provider Services at 312-864-8200, or email [email protected].
