Keeping Your Coverage is Key

The State of Illinois must review your Medicaid eligibility each year.

This process is called redetermination or REDE. If you also receive SNAP benefits, your REDE review is done every six months.

During your REDE period, you will need to complete and return a short form to confirm you are eligible for Medicaid or SNAP benefits. You should receive a letter in the mail from the state with REDE information and instructions about 45 days before your health coverage expires.

You can complete your REDE one of three ways:

  • Online: Go to the Application for Benefits Eligibility (ABE) website and click on “Manage my Case”. If you need help, call us at 312-864-REDE (7333).
  • Fax: Send the completed form by fax to 1-844-736-3563
  • Mail: Mail the completed form to P.O. Box 19138, Springfield, IL 62763

Redetermination Events

CountyCare offers in-person REDE events to help you renew your Medicaid coverage.

At these events, you can:

  • Get copies of your REDE form from DHS to complete and submit your paperwork
  • Learn more about the CountyCare rewards program, care coordination and other community resources like food markets, utility bill assistance, and housing programs
