When you or your children go to the doctor for certain services, CountyCare will send you a CountyCare Rewards Card in the mail to the address that we have on file. The card will have a cash credit on it based on the services you or your children received. Once you activate the card, you can use your rewards to pay for what you need, such as groceries, transportation, utilities, and more.

CountyCare Rewards for eligible services are automatically added to your Rewards Card once we receive information from your doctor’s office. Your doctor has 180 days to send us that information. If 180 days has passed and your reward is missing, please send an email to countycare@cookcountyhhs.org with your name, member ID number, address, and the reward you’d like us to research, or call Member Services at 312-864-8200 / 855-444-1661 (toll-free) / 711 (TTY/TDD).

• All CountyCare members are eligible.
• Members in the managed long-term services and supports (MLTSS) program are only eligible for the annual health risk screening reward.

PCP Annual Visit – $50 reward
CountyCare will give each member 16 months and older a $50 reward for seeing their assigned PCP for an annual checkup.

Annual Health Risk Screening – $50 reward*
CountyCare will give each member a $50 credit on their rewards card once a year for completing a health risk screening. Click here to complete the online form. You may also complete the form by texting “SURVEY” to 84908 or by contacting Member Services at 312-864-8200, toll-free at 855-444-1661, or 711 (TTY). Member Services is available Monday through Friday, 8:00 a.m. to 6:00 p.m.

Colorectal Cancer Screening – $50 reward
CountyCare will give each member ages 45 to 75 a $50 credit on his/her rewards card once a year for completing a colorectal cancer screening from a network provider.

Cervical Cancer Screening – $50 reward
CountyCare will give female members ages 21 to 64 a $50 credit on her rewards card once a year for completing a cervical cancer screening from a network provider.

Pre- and Postnatal Doctor Visits – $50/$10 reward
Pregnant members should see their doctor regularly, including after you have your baby.

• $50 reward for prenatal visits in the first trimester
• $10 reward per prenatal visit after the first trimester, up to 14 visits for the entire pregnancy
• $50 reward for a visit to your doctor 1-12 weeks after you have your baby

Well-child Visits – $10/$50 reward
You can get rewards on your child’s card for taking your baby to the doctor.

• $50 for the first visit in the first month
• $10 for the next five visits

Immunizations for Babies – $10 reward
You get rewards on your child’s card for taking your baby for vaccinations before they turn 2 years old.

• $10 for each immunization (excluding COVID vaccine).

Mammography Program – $50 reward
Women ages 45 to 74 can earn a $50 reward each year when they get their mammogram.

Flu Shot – $75/$25 reward
Members ages 6 to 24 months who get their flu shot from their primary care provider, pediatrician or in-network pharmacy will receive a $75 reward on their card, once a year.
Members 2 years and older who receive a flu shot from their PCP or an in-network pharmacy will receive a $25 reward on their card, once a year.

Notification of Pregnancy– $50 reward
Members who are pregnant can earn a $50 reward when they complete the Notification of Pregnancy form at countycare.com under Member Resources.

Managing Diabetes – $25 rewards
Diabetic members ages 18-75 who visit a CountyCare doctor can receive the following rewards:

• $25 for blood tests and urine screens once a year
• $25 for an eye exam once a year
• $25 for filling a prescription for statin drugs for the first time

Behavioral Health Follow-up Visits – $50/$100 reward
Members who follow up with their doctors after ER visits or inpatient hospital stay for behavioral health will receive the following rewards:

• $100 for seeing your doctor within 7 days after an emergency room visit or inpatient hospital stay for behavioral health; or
• $50 for seeing your doctor within 8-30 days after an emergency room visit or inpatient hospital stay for behavioral health.

Follow-up After Hospital Admission – $20 reward
Members 6 years and older who see a CountyCare doctor within 14 days after an inpatient hospital stay will receive a $20 reward.

COVID-19 Vaccines – $50 reward
Members ages 6 months and older can earn a $50 reward for getting the first COVID-19 vaccine.

HPV Vaccine – $25/$50 reward
Members ages 9-45 who receive both HPV vaccines at least 146 days apart can earn $25 for the first dose and $50 for the second dose.

Meningococcal Vaccine and Booster – $25/$10 reward
Members aged 11 and older who receive the meningococcal vaccine and booster can earn $25 for the first vaccine and $10 for the booster.

Tdap Vaccine – $25 reward
Members aged 10-13 years who received the Tdap vaccine will earn $25.

Care Management Survey – $15 reward
Members enrolled in Care Management who complete the care coordination satisfaction survey can earn $15 each year that the survey is completed.

Controlling Blood Pressure – $50 reward
Members 18 and older who fill the medication prescribed for treatment of their high blood pressure (hypertension) every month for 6 months in a row.

Continuation of Addiction Treatment – $50 reward
Members 13 and older who complete two follow-ups (visit or medication fill) within 34 days after beginning new treatment for a substance use disorder.

Pharmacotherapy for Opioid Use Disorder – $50 reward
Members 16 and older starting treatment for substance use disorder (SUD) who fill their prescriptions used to treat SUD for at least 180 days in a row.

Your Rewards Card is sent to the address that we have on file, and we will text you when new eligible rewards are loaded on to your Rewards Card. You have six months from the date the reward is added to your CountyCare Rewards Card to use the credit. After six months the reward will expire.
