December 10, 2024 – CountyCare Making Special Accommodations to Ensure Continuity of Care for Northwestern Medicine Patients in Medicaid Managed Care
The Illinois Department of Healthcare and Family Services (HFS) is opening a special enrollment period for adults and children currently enrolled in Meridian Health Plan and assigned to a Northwestern Medicine primary care provider.
Meridian Health Plan has terminated its contract with Northwestern Medicine effective December 31, 2024.
HFS’ special enrollment period will be open through January 31, 2025. During this time, impacted members will be able to select a new Medicaid managed care plan that contracts with Northwestern Medicine, including CountyCare, a Medicaid managed care plan operating in Cook County. All other Medicaid members can select a health plan during their annual open enrollment period.
To ensure continuity of care, CountyCare will be waiving all prior authorization requirements for 90 days for transitioning members.
